How to tell what D4xxx phone is?

So according to an app called "Root Toolkit for Android", my phone is a LG D405 L90. See by this screenshot:

However, when I take my battery out, the label where my IMEI number is says LG-D405n.

Do I trust the app? I used this website: http://lg-phone-firmware.com/index.php?firmware=1 to search my IMEI number, and it said my phone was a LG D405n L90. http://lg-phone-firmware.com/firmwar...-Tutti-55.html :

When I first bricked my phone, I followed this tutorial to fix it: http://forum.xda-developers.com/show...43&postcount=9 of course using the D405n loader.img and D405n stock KDZ. When I reboot my phone, I always get this warning:

I super hard bricked my phone, where it only came under Device Manager as "QHSUSB_BULK"/"Qualcomm HS-USB QDLoader 9008" and followed this tutorial http://forum.xda-developers.com/lg-l...iants-t3173429 (Because my phone was already in QHSUSB_BULK/Qualcomm HS-USB QDLoader 9008 mode, I didn't have to disassemble my phone).

My phone is rooted, with the TWRP recovery and bootloader unlocked.

I have made this thread to see if anyone could possibly help me find out what kind of D4xxx my phone really is. Thank you.
